Improving Care Transitions through Bundled Payments /

Healthcare leaders are increasingly concerned about the impact of unplanned rehospitalizations and how to best manage patients’ transition from a hospital to another care facility. Current healthcare reform bills call for the creation of a pilot program that would bundle payments for hospital and post-hospitalization care services as part of efforts to improve care coordination and control costs. Both proposals give the Centers for Medicare and Medicaid (CMS) considerable flexibility in the pilot’s design, leaving open a number of important policy considerations that have the potential to reshape Medicare spending, patient care, and the relationships between hospitals, physicians, post-hospital care providers, and patients.

Avalere recently completed a self-funded analysis that examines this new federal focus. It looks at the type of services included in a bundled payment and how to account for varied medical conditions when deciding the time period a bundled payment will cover between a hospital and a post-discharge care location.
